In consideration of Clearview Baptist Church (“Clearview”) providing me with the opportunity to travel and participate in various mission projects that are sponsored in whole or in part by Clearview (“Mission Projects”), including those Mission Projects held locally, and those held internationally, I agree and acknowledge on behalf of myself, and any minor child for whom I am the parent or legal guardian listed below, as follows:

1. I hereby acknowledge and agree that Clearview is providing me and/or my minor child with the opportunity to travel and participate in various Mission Projects, and I recognize that my participation in these Mission Projects puts me at risk of serious bodily injury and possible death. I acknowledge and agree that no matter how careful Clearview, its affiliates, partners, and employees are, these risks cannot be eliminated. Being fully aware of these dangers, I acknowledge and agree that my and/or my minor child’s participation in the Mission Projects is entirely voluntary. I hereby acknowledge and agree that should I and/or my minor child suffer any damage or injury while participating in the Mission Projects, including but not limited to traveling to and from mission sites, tutoring, mentoring, teaching and serving individuals in communities, engaging in manual labor, and participating in other activities that may take place in dangerous or difficult conditions, that all risk and responsibility is solely my liability.

2. Based on the foregoing, I hereby assume any and all risks incidental to and arising from my and/or my minor’s participation in the Mission Projects, and that Clearview is not responsible for any actions, inactions, or results associated therewith.

3. I hereby acknowledge and agree that I will indemnify and hold harmless Clearview, related and affiliated entities, and the directors, employees, agents, volunteers, representatives, successors, and assigns, from and against all allegations, claims, actions, suits, demands, damages, liabilities, obligations, losses, settlements, judgments, costs and expenses (including without limitations attorneys’ fees and costs), which arise out of or result from any act or omission on my and/or my minor child’s behalf, related to my and/or my minor child’s participation in the Mission Projects.

4. The Released Parties are: Clearview, related and affiliated entities, and the officers, directors, employees, agents, representatives, successors and assigns of each of the foregoing entities. On my own behalf and on behalf of my minor child, if applicable, I (1) release and forever discharge the Released Parties of and from all liabilities, claims, actions, damages, costs or expenses of any nature whatsoever for bodily injury (including death) and property damage arising out of or in any way connected with my and/or my minor participation in the Mission Projects; and (2) indemnify and hold each of the Released Parties harmless against any and all such liabilities, claims, actions, damages, costs or expenses, including, but not limited to, all attorney's fees and costs. I expressly agree that this release and indemnity agreement includes, without limitation, any claims based on the negligence, action or inaction of any of the Released Parties, and covers bodily injury (including death) and property damages suffered by me and any minor child for whom I am the parent or legal guardian, while participating in the Mission Projects.

5. I hereby release the Released Parties from any and all liabilities, claims, actions, damages, costs, or expenses of any nature whatever for bodily injury and property damage arising out of the actions of another participant.

6.I agree that I am fully and solely liable for any and all medical expenses I and/or my minor child incur based on any injury incurred participating in the Mission Projects. I hereby authorize medical treatment for me and/or my minor child if the need arises while participating in the Mission Projects.However, I acknowledge and agree that the Released Parties shall have no duty, obligation or liability arising out of the provision of or failure to provide medical treatment if incidents occur while participating in the Mission Projects.

7.I agree that any claim which I may bring forward at any time for any reason against the ReleasedParties shall be governed by the laws of Tennessee and the parties submit to the exclusive jurisdiction and venue in the courts of Williamson County, Tennessee, and none other.

8.I am not under the influence of alcohol or drugs, and there is no impairment or imbalance that would affect my understanding of all the terms of this Agreement.
